ANALYSIS OF STATE MANAGEMENT OF THE SYSTEM OF PROVIDING PALLIATIVE AND HOSPICE ASSISTANCE FOR THE POPULATION OF UKRAINE IN THE PRE-WAR PERIOD

The article analyzes the state management of the system of providing palliative and hospice care to the population of Ukraine in the pre-war period (until February 24, 2022). It was found that the state management of the system of palliative and hospice care in the pre-war period was focused on the Ministry of Health and the Ministry of Social Policy of Ukraine. It has been established that palliative and hospice care in Ukraine is provided free of charge to incurable (incurable) patients according to medical indications and in accordance with the regulations and legal acts. Forms of providing palliative and hospice care based on the qualifications of the subjects providing such care and the complexity of the morbidity of incurable patients have been defined and substantiated. Gaps in state administration in the system of providing palliative and hospice care in the pre-war period have been revealed, in particular, firstly, the staff lists of hospices and other institutions that provide assistance to incurable patients did not include the positions of social worker, psychologist, legal consultant, etc. which does not make it possible to ensure the implementation of a multidisciplinary approach to the provision of such assistance; secondly, the issue of developing a network of hospice institutions (departments, rehabilitation centers), their creation and operation, and the provision of palliative care was entrusted to the regions, which, in the conditions of insufficient development of the financial mechanism of state management of palliative and hospice care, constant underfunding of such care measures from the state , local budgets are resolved extremely unsatisfactory. In light of recent events, 80% of Ukrainians are forced to die at home due to lack of funds to pay for treatment in medical institutions; thirdly, there are no effective state coordination mechanisms for interdepartmental and interdisciplinary cooperation in providing such assistance at the central and local levels; fourthly, there was no system for training medical, social and other workers in the specialty "palliative care" (which was not in the list of specialties) and improving their qualifications; fifth, a low level of public awareness of palliative care (3% of the population does not understand what "palliative care" is) combined with low activity, participation of non-governmental organizations and philanthropists in providing such care; sixth, low levels of quality and volume of palliative and hospice care, which is a natural integrated result of the problems listed above.
